Samenvatting (Frans of Engels)
Het snel toenemende aantal ouderen (≥ 65 jaar oud) met traumatisch hersenletsel (TBI) heeft aanzienlijke medische en economische gevolgen. Een intracranieel hematoom, en meer specifiek een acuut subduraal hematoom (ASDH), is het meest voorkomende letsel bij ouderen met TBI. De chirurgische versus conservatieve behandeling van deze patiëntengroep blijft een belangrijk klinisch en moreel dilemma, omdat het in veel gevallen onduidelijk is welke behandeling tot een beter resultaat voor de patiënt leidt. De huidige richtlijnen zijn niet gebaseerd op kwalitatief hoogstaand bewijs en de naleving ervan is laag, wat zorgt voor grote variaties in behandeling, in België en Nederland. Daarom is er behoefte aan een prospectieve, gerandomiseerde, multicenter-studie naar de (kosten-) effectiviteit van vroege chirurgische hematoomevacuatie versus een conservatieve behandeling bij ouderen met een traumatische ASDH.

Patients ≥ 65 years with at first presentation a GCS ≥ 9 and a traumatic ASDH >10 mm or a traumatic ASDH <10 mm and a midline shift >5 mm, or a GCS < 9 with a traumatic ASDH <10 mm and a midline shift <5 mm without extracranial explanations for the comatose state, for whom clinical equipoise exists regarding the preferred treatment. |
Intervention (I) |
Early surgical hematoma evacuation (A). The exact neurosurgical technique will be left to the discretion of the surgeons. |
Control (C) |
Conservative management on the ICU or the ward (B). In case of neurological deterioration during conservative management, delayed surgery can be performed. |

Functional outcome after 1 year, expressed by the rating on the Extended Glasgow Outcome Scale (GOS-E) |
